- Consider the following guidelines when developing your nomination:
- Describe the nominees contributions to the advancement of health innovation in Alberta;
- Describe how the nominee has demonstrated leadership in their respective domain;
- Describe the nominees impact, value, and tangible benefits to advancing health innovation in the province;
- Describe the nominees commitment to excellence in advancing health innovation;
- Any other information that supports this nomination;
- 2 Letters of Support (required for submission)
